Transfer of Medical Records
The real story is that a summary of any given patient’s medical records can be insufficient information to care for the patient involved.
For that reason, our doctors will let you know if we need medical records transferred from alternate service providers, such as a doctor or hospital, that you have been to in the past.
We ordinarily charge an administration fee for transferring our medical records to somewhere else.
Similarly, you may want our medical records sent somewhere else.
- For security reasons, we cannot accept a request to transfer medical records over the internet.
- Transferring our medical records to an alternate service provider usually means visiting our medical practice, and completing our “Transfer of Medical Records Request Form”, to meet our security and privacy concerns.
